Welcome to the Golden Plains Health and Wellbeing partner hub. This page is intended to act as a platform to support partners through information sharing and as a portal for accessing relevant documents and resources in a convenient central location.

Golden Plains Shire (GPS) Council works closely with health and wellbeing partners to support the delivery Council’s Municipal Health and Wellbeing Plan (MPHWP). Underneath the overarching MPHWP is a regional Health and Wellbeing Action Plan 2017-2021, which aims to capture the collective health and wellbeing actions under five health and wellbeing priority areas:

Our approach is highly collaborative, with the collective involvement of over 30 external organisations (Community Health organisations, Primary Care Partnerships, Sport Assembly agencies, non-for-profit health services) and 15 internal Council Departments/Units (Maternal and Child Health, Recreation and Youth) all of whom have actions within the regional plan. 

Consistent with direction from state government and other funding agencies, we are striving to work collectively with an emphasis on the importance of coordination in planning to achieve better health and wellbeing outcomes for community members across Golden Plains Shire Council.

To support this regional approach a need was identified by the Council's Health and Wellbeing team for the development of a consistent and clear approach to support Evaluation and Monitoring of our regional plan. 

In April 2018 Council's Health and Wellbeing team commenced the process of developing an Evaluation and Monitoring Plan along with a supporting Evaluation Toolkit. This Toolkit can be used by our health and wellbeing partners to capture learnings and progress of our collective health and wellbeing contributions.

Moving forward over the next 12 months (2019-2020 trial)

We are excited to be trialling data collection regionally to capture the work from our health and wellbeing partners. This will allow us to review collective actions, and eventually link actions to Intermediate (impact) and Long Term (outcome) measures.

Golden Plains Shire Data Profile 2019

The Golden Plains Data Profile has been updated for 2019-2020. This is a collation of datasets with measures relating to Golden Plains Shire's demographics, health and wellbeing as well as economic indicators among other metrics. 
Data has been sourced from across the four environments identified by the Victorian Government's 'Environments for Health' framework for public health planning, as impacting on health - social, economic, natural and built environments. This approach promotes the importance of incorporating health planning into whole-of-council planning.
